Registration

Please download the following two (2) files to your computer. First, fill in the excel file with your roster and please make sure all of the information is accurate and complete. Second, download and make copies of the Mid-Summer Classic Lacrosse waiver form. Every player must have this waiver completed and signed before the first day of competition.
